Requirements for Correction of Entry in the School Records and/or Diploma

1. Letter-request for Correction of Entry signed by the requestee specifying therein the entry/entries to be corrected, addressed to :

                BENJAMIN D. PARAGAS, CESO V

                    Director IV

                 Office of the Regional Director

2. Valid Identification card with picture (i.e. Passport, PRC/Driver’s License, UMID/SSS, Seaman’s Book, etc.)

  * preferably with the data to be corrected (i.e. date and/or place of birth)

3. Certification from the school as to date of graduation with School Seal

  * or as to last school year attended/completed (for undergraduates)

  * or as to present admission status (for students currently enrolled)

4. Original copy of Birth Certificate issued by the Philippine Statistics Authority (National Statistics Office)

  * With copy from the Local Civil Registrar if unreadable/unclear.

5. Notarized Joint Affidavit of two (2) disinterested persons (not related to the applicant)

6. Original copy of Form 137 with School Seal

7. Diploma (especially for correction of name)

8. List of Graduates issued by the School with School Seal.


If the applicant has a duly authorized representative, the Representative must bring the following: (IN ADDITION to numbers 1- 8 enumerated above)

          1. Authorization letter; and

          2. Valid Identification card of the representative


* ALL REQUIREMENTS MUST BE SUBMITTED IN ORIGINAL WITH TWO (2) PHOTOCOPIES/XEROX EACH (preferably certified by the school)
